I hereby agree to indemnify and hold harmless Pro Skills Basketball, LLC its parent, subsidiary and affiliated entities and/or any officers, partners, members, directors, coaches, employees, servants, agents, licensees and assigns of any of the foregoing, from and against any and all suits, awards, claims, damages, liabilities, costs and expenses (including reasonable attorney fees and related costs) arising out of injury or damages to my child in connection with his/her participation in any Pro Skills Basketball program. I hereby authorize Pro Skills Basketball, LLC to act for me according to their best judgment in any medical emergency situations.

* NOTE: I agree that any picture(s) and/or video(s) taken of my child by Pro Skills Basketball, LLC during this function are the property of Pro Skills Basketball, LLC and may be used for any promotional and/or advertising purposes by Pro Skills Basketball.

* NOTE: All deposits are non-refundable and no refunds will be given for cancellation of an event by participants if Pro Skills Basketball is notified 48 hours or less before said event is set to start. No refunds will be given for missed days, classes, trainings, or any other missed time of an event.

*NOTE: I understand that my child's place in an event it not reserved until I have either paid the full price or the non-refundable deposit for said event.
